|Primary Purpose: |To undertake complex assessments to ensure that an appropriate range of identified services |

| |are provided to a designated person or group of people within an agreed budget managed by |

| |the post-holder. |

| |May supervise staff within a team. |

|Main Duties and Responsibilities: |

|Following referral, to establish the client's eligibility for assessment and determine the level of assessment required. |

|To undertake complex assessments in conjunction with the client and/or their carers. |

|Together with the client and/or their carers, to produce a care plan to meet the needs of the client which takes account of |

|Departmental policies, guidelines and procedures. |

|To use the existing market place in order to obtain the most appropriate and cost effective supplier of services consistent with the|

|needs of the client. |

|To ensure that the agreed care plan can be provided within the available budget. |

|To monitor the care being provided to ensure that needs are continuing to be met. |

|To re-assess clients’ needs periodically and, when appropriate, to amend the care plan following consultations with the clients |

|and/or their carers. |

|To provide on-going support and advice to clients and their carers. |

|To identify areas of need which cannot be met and to systematically record and report the unmet need to the Team Manager. |

|To bring to the notice of the Team Manager any instances of perceived failure of a provider to deliver the agreed level of service. |

|To undertake duties under the Mental Health Act as appropriate. |

|To maintain all necessary records and documentation in line with Departmental administration procedures and use computerised |

|information systems. |

|To take part in the Team Duty System where appropriate. |

|May be responsible for the supervision and appraisal of a number of staff within a team. |

|To assist with the production of standards of practice and training and development activities. |

|To take part in working groups, sub-groups, etc. both within and outside the Department. |

|To assist with the production of quality standards and performance measures and to ensure that they are adhered to. |

|To undertake such other appropriate responsibilities as may be delegated from time to time by the Team Manager. |

|To comply with all health and safety guidelines. |

SOCIAL WORKER

Career Grade Progression - Range 5 - 7

All career grade appointments will be made to one Range and the offer of appointment will state that advancement to a higher Range is not automatic but is dependent upon review. The review will encompass a minimum Range-by-Range requirement as set out below and will be subject to the following criteria.

a) That sufficient work must be available at the higher Range.

b) That there is evidence that the employee will be able to undertake work at the higher level.

c) That the employee must have shown proper motivation, conduct and standard of performance in respect of past and current duties and responsibilities and have made realistic efforts to achieve all targets set.

Progression and appointments within the Career Grade may be at any point within the relevant Range and at the discretion of the Team Manager, with approval by the relevant Service Manager and final approval by the relevant Head of Service, to take account of qualifications, experience and contribution of individuals.

Minimum Range by Range Requirements

Range 5 will be applicable to staff with relevant qualification e.g. CQSW, CSS, Diploma in Social Work. Following successful evaluation after a minimum of one year’s post-qualifying experience, staff will progress to ISN 28.

• Take responsibility for a number of cases, developing a caseload of more complex cases

• Work to an agreed programme

• Formulate appropriate care plans

• Negotiate “one-off” contracts with minimum supervision

Range 6 will be applicable to staff with a minimum of two years appropriate experience following two successful evaluations.

• Managing a large and complex caseload

• Full range of social work duties including complex assessments and negotiation of “one-off” contracts

• Supervision and coaching of more junior staff

Range 7 will be applicable to staff with at least two years post qualification and/or relevant pre-qualification experience, following three successful evaluations and who meet all the essential criteria set out in the person specification.

• Commitment to CPD

• Completed all appropriate vulnerable adult training

• Management and progression of complex cases

• Leading on an area of work

• Chairing meetings e.g. core groups, investigations, complex case conferences

• Contribute to/ lead working groups

• Evidence of knowledge in specialist area e.g. difficult family dynamics etc

• Evidence of effective use of IT systems

• Supervision of assigned staff

All Ranges - Criteria for accelerated progression, i.e. more than one increment annually. Normally considered annually at appraisal in the context of exceptional performance of the competencies identified for this particular role. One or more of the following criteria must be satisfied. Acceleration is subject to the approval of the Head of Service.
