To work within a community based team to provide a social work service to adultcustomers and their carers, working jointly as part of an integrated Multi-disciplinary team with other agencies in undertaking assessments, care planning and review of customer’s needs, specifically using the Care and Treatment planning model and a Recovery based, solution focused approach.? To promote the independence of customers of Managed care and SafeguardingServices and protect those who are most vulnerable by ensuring that the tasksassociated with assessment and care management are undertaken in acustomer-focused, resource efficient and timely manner.PRINCIPAL ACCOUNTABILITIES1. To assess, and/or co-ordinate the assessments of the strengths and needs ofusers and their primary carers using the established assessment format,together with the department's criteria for service provision within thelegislative and regulatory framework.2. Develop and cost packages of care which are flexible, cost effective andresponsive to the changing needs of the individual. Promote independence,self determination and the maximum choice through access to a range ofservices provided by the independent and statutory sectors.3. To undertake specialist social work care planning including the provision of directspecialist social work interventions. e.g. AMHP role and task as prescribed by the MHA 1983 and Codes of Practice.4. To contribute to the management of the community care budget and themaintenance of expenditure within set budget limits.5. To ensure that individuals have information made available to them in an appropriate format which enables them to make informed choices.6. To actively work in partnership with colleagues from other agencies.7. To review and/or co-ordinate the review of care packages on a regular basis andfacilitate changes as necessary.8. To identify service deficiencies and pass this information to the line manager.9. To participate in all training opportunities as deemed appropriate.10. When two years post qualified, undertake to qualify and take on the role of practiceassessor and/or practice facilitator for social work students on placement.PERSON SPECIFICATION1. Social Work qualification (CQSW/DipSW/CSS/BaHons/Masters).2. Approved Mental Health Professional qualification or the commitment to undertake AMHP training as a priority.3. Practical knowledge of legislative framework – including the Mental Health Act 1983 and amendments, Mental Capacity Act 2005, Social Services and Wellbeing Wales Act 2014, Mental Health (Wales) Measure 2010 and other relevant legislation.4. Ideally, considerable post qualification experience with the client group.5. Sensitivity and approachability.6. Clear/concise written and verbal communication skills.7. The ability for self organisation and caseload management.8. Ability to work positively with other agencies.9. Good IT skills with a thorough working knowledge of the Microsoft suite of software.10. Qualified as a practice assessor or prepared to undertake the training.11. Valid driving licence and car owner. ................
