POSITION TITLE:

Mental Health/Social Worker (MH/SW)

REPORTS TO:

Wawa Family Health Team Executive Director

DEFINITON AND STATUS:

This is a full time, permanent position with the Wawa Family Health Team.

The MH/SW will function as a member of the primary care team in the provision of counseling therapy and other therapeutic functions in the Wawa Family Health Team. The MH/SW provides services to clients who include the individual throughout their life span, the family, and the community with a strong emphasis on health promotion, patient education and disease prevention.

The MH/SW provides primary care functions and participates in the development, implementation, monitoring and evaluation of programs and services of the Wawa Family Health Team.

REQUIRED KNOWLEDGE AND COMPETENCIES:

✓ Education/Experience

● Bachelor’s Degree in Social Work or a Social Science from a recognized

University;

● Three to five years experience in counseling/therapy preferably in a community setting would be an asset;

● Registration with the Ontario College of Social Workers and Social

Service Workers;

● Working knowledge of the Mental Health Act, Healthcare Consent Act,

Substitute Decisions Act and community mental health resources;

● Current Ontario Driver’s license

● The Social Worker is to practice within the scope and according to the standards of practice as outlined in the following documents:

- College of Social Workers and Social Service Workers

- Regulated Health Professions Act (1991)

● Function within human resources and guidelines of FHT policy and procedures;

● Respect the dignity, worth and rights of each individual receiving care.

✓ Skills/Abilities

● Proficient in assessment, psychotherapy and psychological counseling techniques for working with individuals, couples, families and communities;

● Good time management, communication and interpersonal skills;

● Knowledge and proficiency in current, evidenced-based methods and practices of primary care delivery, with an emphasis on health promotion and risk reduction;

● Proficiency in the use of the computer hardware and software, particularly in Microsoft Word, Excel, Outlook, experience using an electronic medical record preferred;

● Desire and ability to update knowledge and skills through various means including technology-based opportunities, courses, workshops and conferences;

● Exhibit an ability to be open and non-judgmental;

● The MH/SW maintains knowledge of the goals and structure of the organization including the policies, procedures and programs offered;

● Superior communication, problem solving and conflict resolution skills;

● English required; French not required but would be an asset.

As the WFHT is a newly developed organization the roles and responsibilities of this position will evolve and develop over time. The Mental Health/Social Worker needs to be flexible and able to adapt as the organization progresses.

DUTIES AND RESPONSIBILITIES

Clinical:

1. Provides counseling/therapy and other therapeutic functions for individuals, couples, families and the community;

2. Screens, prioritizes and processes client referrals;

3. In collaboration with members of the team, develop and discuss appropriate individualized care plans with the client/family/caregiver based on best practices;

4. Works effectively within an interdisciplinary team, in varied practice settings, to improve access to primary care for the residents of Wawa and area. The “client” may be an individual, a family, a target group or a community.

5. Recommends and adapts teaching to the client’s identified health needs, revising treatment protocols as required;

6. Facilitates group educational sessions and activities using the principles of child, youth, adult or older adult learning as required;

7. Stays current and aware of opportunities to implement new, evidenced-based methods of client assessment, treatment and programming;

8. Participates in clinical projects / studies as required;

9. Models the values and philosophy of the organization;

10. Communicates effectively with all clients, families, peers, other health care professionals and community partners;

11. Facilitate coordinated access to services in areas such as assistance with daily living, crisis intervention, treatment health promotion and prevention;

12. Advocate for client’s civil and legal rights.

Administrative:

1. Maintains strict confidentiality of team, personnel and client information in all aspects of work;

2. Actively participates in the development, implementation, evaluation and maintenance of the FHT’s programs and services in partnership with the RN program Manager and other A.H.P.’s;

3. Communicates effectively with health care team members to create a cohesive team and seamless services to the community;

4. Actively participates in staff, team and committee meetings and reads associated minutes as required;

5. Documents using client health record, refers to other resources and arranges follow-up as necessary.

6. Participates in interdisciplinary committees to promote professional / interpersonal development;

7. Maintains and develops professional competence through ongoing professional development;

8. Participates in self-directed learning to ensure that practice remains relevant by attending professional conferences, e-learning and journal reviews;

9. Exercise professional judgment in the absence of WFHT procedures, protocols or position statements;

10. Clarifies the social worker’s role and responsibilities to other health care team members (e.g. with regulated and unregulated health care providers);

11. Proficient in computer skills and uses IT in as many aspects of practice as possible;

12. Promotes the Wawa Family Health Team mission statement within the organization and works to achieve goals and objectives as specified in the organization’s strategic plan;

13. Promotes awareness of the FHT’s services and programs;

14. Participates in the orientation process;

15. Participates in the Performance Development Process with his/her supervisor every year or as required;

16. Promotes a safe and healthy working environment;

17. Participates in recruitment/retention initiatives; including participating on hiring committee when requested.

18. Co-operates with all programs working closely with the Wawa Family Health Team; including the Wawa Medical Centre, Algoma Public Health, Lady Dunn Health Centre, etc. Has knowledge of other agencies as it relates to the MH/SW’s position;

19. Assists and works collaboratively with Wawa Medical Centre personnel and medical staff as required.

20. Participates in team-building initiatives within the organization;

✓ Reporting Relationships

Prepare monthly or as required statistical reports for the Ministry of Health and Long Term Care or as required by the Wawa Family Health Team.

✓ Nature Of Supervision Received:

The MH/SW position will receive direction from the Wawa Family Health Team Executive Director. This position requires self-direction and independent work. Self-organization and flexibility is essential to complete necessary priorities in a timely manner. A positive attitude and creativity is required as there will be many ongoing changes and challenges as the Wawa Family Health Team adapts and advances.

Ongoing communication with the Executive Director and Wawa Family Health Team members is essential in order to ensure information is shared appropriately.

Teamwork is required to work effectively with other professionals and representatives of other groups and organizations in the community.

All hours and administrative protocols are clearly defined in the WFHT Policy Manual.

• Work in a standard office environment.

• Required to handle multiple priorities.

• Required to handle confidential matters.

• Required to attend to limited local and out-of-town travel, use of own vehicle and valid driver’s license is sometimes required.

✓ Physical Demands:

Moderate: The position requires minimal physical effort and the workday comprises roughly of equal amounts of standing and sitting, with frequent position changes.
