MPS: Behavioral Referral

The Behavior Referral Form allows school staff members to complete an online referral for a student behavior event and submit it to an administrator responsible for managing behavior for further action. To access the behavioral referral, login into Infinite Campus, found in APPS, with your Milwaukee Public Schools username and password

When you log into Infinite Campus, you want to be on the Campus Tools section. If you are on the Campus Instruction section, then go to the Application Switcher to select Campus Tools.

The Application Switcher will open a popup window, which will allow you to select Campus Tools.

You will now see that you have switched to Campus Tools.

Once you are at Campus Tools, locate and select the Behavior tab on the left side of the page, found under the Index.

Creating a Behavior Referral Select the Behavior tab, than select Behavioral Referral. To create a new referral, select New to open a new Behavior Referral and begin to enter information for the incident in the Incident Detail Editor.

Incident Detailed Information Enter information into each field or drop down. At any point while creating a referral, by clicking Save Draft for the Behavioral Referral or Event/Participant editor, all information will saved in draft status.

Leave the Title blank. It will be automatically entered later with the event you choose for the incident

Context and Location MUST be filled in for PBIS. (Context Description and Location Description are optional.) .

In the Incident Detailed Information, only general information about what happened, where it occurred, the time/date that it occurred and the administrator/s are entered.

No students should be identified in the Details section. (Ex: Student 1,

Student 2, He, She, etc.) This can be seen by parent who wants to view the referral. Specific student information, concerning their responsibility in the incident is entered in Add Event/Participant. To add information concerning the incident and specific student information, select the Add Event/Participant tab at the bottom of the Incident Detailed Information.

Add Event/Participant The Event Type is the incident or incidents in which the students were involved. There is no limit to the number of events that can be added to an incident.

From the Event Type drop down, select the type of event that you believe best describes what occurred. Behavior Response is only checked if a physical restraint was used by a staff member during this incident. Participant(s) Detail Under Add Participant, choose the filter you need to use to search for and add each participant for the incident. A participant can be a student or school employee.

To attach a participant to an event, Search for at least the first two characters of the Participant Name or a Student Number who are part of the active enrollment or school calendar.

Once each participant has been selected, choose a Role for each participant. A participant's Role can be an Offender, Participant, Victim, or Witness. For each participant, write a detailed statement in the Details section. The only name that should be used in the Details section should be for that specific participant. If a parent/guardian requests to see the referral, only their child's name should appear.

To return to Incident Detailed Information click Save Draft.
