Senior Program Manager, Community Health, Kaiser Permanente

SUMMARY Leads strategies that measurably improve health and wellbeing in underserved communities. Develops and implements strategies that result in health care services, public policies, practices, and programs that effectively address identified needs. Strong knowledge of and ability to support the measurement, assessment, and articulation of the impact of community health strategies. Leads and coordinates effective collaboration with external partners, including community residents, non-profits, for-profits, government agencies, schools/school districts, and health care organizations. Engages internal stakeholders to align community needs and business imperatives, and identify and deploy organizational resources that support community health improvement. Guides grantmaking and other contributions.

MAJOR RESPONSIBILITIES/ESSENTIAL FUNCTIONS

? Strategic Planning and Implementation: Leads and/or supports the strategic planning, implementation, evaluation, and

funding of community health programs. Gathers and reviews data to inform program development, grants, and

measurements. Collaborates with stakeholders on program development and implementation, policy advocacy, events,

community engagement and other initiatives. Develops and maintains strategic partnerships with community, government

and other stakeholders to address medical needs and social determinants of health in communities. Works collaboratively

with regional and national Community Benefit teams to develop innovative approaches to population health

improvement. 45%

Stakeholder Engagement: Builds and maintains relationships with community leaders including non-profits, for- profits, elected officials, public agencies and other key community stakeholders to engage in strategic community health work. Serves as a knowledgeable representative of KP on collaboratives and working groups. Engages Regional and Program Office leadership and other key internal stakeholders to support execution of community health strategies. Represents KP on coalitions, working groups, advisory boards, committees, panels, planning groups, etc. Provides assistance to these groups on topics relevant to health care, policy and systems change strategies for population health improvement. 25%

? Grant making and other contributions: Identifies resources to support community health initiatives. Contributes to RFP development, reviews grant proposals and recommendations for grants committee and national board of directors. Participates in site visits and provides technical assistance to grantees. Develops strategies for authentic community engagement and connect community organizations, as appropriate, to grant and sponsorship opportunities. 15%

? Communications: Collaborates with internal and external leaders to gather insights on opportunities to make community

health initiatives more impactful. Leverage insights and relationships to identify and operationalize enhancements that

optimize investments. Ensures meaningful reporting and regular communication. Pursues existing and new awards and

recognitions aligned with Kaiser Permanente's community health efforts. Supports CB evaluation efforts through identifying

themes and trend that inform new strategies and decision making. 10%

JOB QUALIFICATIONS Minimum Education

? Bachelor's degree in Public Health, Health Policy, Social Work or related field OR four (4) years of experience in a directly related field.

? High School Diploma or General Education Development (GED) required.

Preferred Education ? Masters Degree in Public Health, Health Policy, Social Work or related field.

Minimum Work Experience and Qualifications Basic Qualifications:

? Minimum seven (7) years of combined relevant experience in the areas of community based public health initiatives, policy, systems, environmental change strategies, and healthcare.

? Proven success at building and maintaining relationships with community-based organizations, community coalitions/collaborates, and networks.

? Solid written and oral communication skills that demonstrate cultural competence and ability to work effectively with a wide range of internal and external stakeholders.

? Knowledge of policy approaches to public health improvement. ? Existing and emerging knowledge of best practices in organizing and implementing community health policies/programs.

Proven project management skills, and ability to maintain high productivity amid competing priorities. ? Proven ability to exercise sound judgment under pressure. ? Ability to prioritize and manage own work; exercise independent judgment, initiative and motivation. Proven ability to think
